Are you passionate about providing high quality nursing care to people in their own homes and want to support individuals to remain in their preferred place of care, preventing unplanned admissions, then the Out of Hours Nursing Team is for you.

The team provides a service from 14.30 – 08.00hrs worked over various shift patterns, supporting our population across Exeter, East and Mid Devon, but based in central Exeter.

We are currently looking to further develop our out of hours service utilising skills to deliver responsive, timely, quality care to meet the needs of our population. You will be able to demonstrate your enthusiasm and commitment to community nursing, and further develop your clinical skills in meeting the needs of our patients out of hours. The post-holder will be supported by Band 6 nurses, the Team Manager and a Senior Nurse.

Our Out of Hours Nursing Team within the Health and Social Care Community Team consists of nurses, healthcare assistants and support workers (Roving Responders). The team are developing closer links with the day community nurse teams to provide an element of planned care and to support the needs of our populations around the clock, also undertaking responsive holistic nursing and social care as required. The caseload covers a broad range including end of life care, admission avoidance and support for timely hospital discharge. The team also provide clinical support for our Urgent Response support workers between the hours of 5pm – 11pm.

Applicants will be required to commit to all shift patterns within the team and be comfortable with driving, sometimes long distances across the geography.

Experience in community nursing is desirable for these posts but not essential as full training will be provided and the post holder will be supported to develop in the role. There will also be many opportunities to develop and progress your career.
