Urgent Community Response is a joint Health & Social care service providing support to people in their own homes in times of crisis, to avoid unnecessary admissions to hospital and facilitate discharges home from hospital. Hours are fully flexible over 7 days a week between the hours of 07:30 & 22:00. The service provides care across the Mid Devon communities and you will be part of the locality team in Tiverton & Cullompton providing a quality service promoting independence, dignity & support. This support also extends to stroke patients within our locality. Our service aims to avoid admission for patients who could safely be maintained and supported at home as well as enabling people to return to their home as quickly as possible from hospital.
Hours and shifts are offered on a full or part time basis, including weekends, bank holidays and evenings at enhanced rates. Substantive, Fixed term & Secondment contract opportunites are available.
Previous community experience is desired but not essential as the teams will give you the support and guidance you need to excel. As this isa community based position, a driving licience is essential and access to your own vehicle.
You will join an enthusiastic and dynamic multi-disciplinary team, working with Community Nursing and Therapy teams to respond to urgent situations, to enable people to stay in their own homes or ensure a timely discharge from Acute & Community hospitals. You will be provided with a comprehensive induction programme including support to complete the National Care Certificate.
Successful candidates will have NVQ3 in Health & Social Care and ideally have experience of working with older people in a community setting.You will also need to be competent with IT as electronic recording is an essential part of the role. UCR support workers are lone workers within the community, although support is always available via Team Managers and the Community Nursing teams.
